Please print out and complete either the Child or Adult Information sheet and bring it to your first appointment. You may also complete these forms online, save them to your device, and send them to the office at info@ICTPsych.com You may also print out the Acknowledgement Notice, Notice of Privacy Practices, and the Office Policies.
