Do not be frightened by the length of the Vanderbilt checklist in this link.  It is easy to fill out.  The checklist is divided into several parts.  The first is the most important, and is used to assess your child's symptoms, when you are seeking help for your child suspected of ADHD.  There is a teacher checklist that is optional, and follow up checklists intended to track progress in symptoms and any medication side effects.
